Helpful Workers' Comp Forms & Info
After you sign up for ECOMP, you must fill out one of the following:
CA-1: Traumatic Injury Report Form: Should be used if you have sustained a traumatic injury on the job. A Traumatic Injury is a wound or other condition of the body caused by external force, including stress or strain. The workplace injury must be identifiable by time and place of occurrence, member of the body affected, and must be caused by a specific series of events or incidents within a single day or work shift.
CA-2: Occupational Disease Report Form: Should be used if you have sustained an occupational disease injury on the job. An Occupational Disease is a condition produced in the work environment over a period longer than one work day or shift. It may result from systemic infection, repeated stress or strain, exposure to toxins, poisons or fumes, or other continuing conditions of the work environment.
Once established with a clinic, you will be assessed by our medical doctor and we will help provide additional documentation to assist in your claim:
CA-17: Duty Status Report: A form completed by your treating physician that outlines the diagnosis, physical limitations and what work you can or cannot do. It translates medical findings into work capacity terms and allows your agency to offer light or modified work duty. The CA-17 is often the bridge between medical and employment decisions.
CA-20: Attending Physician's Report: This form provides OCWP with a medical diagnosis, explaining the causal relationship between your condition and your federal employment. It includes objective findings and medical reasoning and if completed incorrectly or not completed, is often times the reason most denials in cases happen. Not every case requires a CA-20, most cases only have a CA-17.
